Thank you for your reservation. This form will help us create the best schedule for your visit. Please fill it out carefully and completely. The top portion asks for basic information and is required. After filling out the top portion completely, make your selections for tours and programs and hit the submit button. All information received through this form is kept confidential and will not be shared with other parties.

Museum Tours

When checking off your tour and program selections please check your first choices. If we are unable to schedule these choices you will receive an e-mail or phone call with alternative suggestions. Remember, large groups will be divided into groups of approximately 15 people. Most tours and programs are a half an hour unless otherwise stated. Blaine House and State House tours are free. Museum gallery programs and tours are free for K-12 educational groups. There is a $5.00 per program fee for all other groups.

After you hit submit, an e-mail acknowledgment will be sent letting you know the reservation request has been received. Shortly afterward you will receive an e-mail with a draft of your schedule or a phone call. When a finalized schedule has been worked out a confirmation package will be sent with additional information. We are looking forward to your visit!
